The challenge

The customer developed a novel therapy and planned its launch. This treatment introduced a completely new operational process, as it had several critical constraints:

  • The therapy product could only be administered once per patient
  • The manufacturing process for the new treatment material was time-sensitive, with a 24-hour shelf life starting from final preparation
  • Each treatment material was highly personalized, being prepared for a specific patient
  • The treatment material was highly expensive, making any spoilage a significant financial risk

Therefore, the new therapy required real-time synchronization of workflows and activities between multiple stakeholders, including treatment facilities, manufacturing hubs, logistics partners, and the company's internal teams. Real-time tracking and immediate notifications of any changes were essential. Therefore, the organization needed a new digital solution for an end-to-end therapy management process, capable of orchestrating medical and operational workflows in real time - from obtaining patient-specific data from EHR systems of a treatment facility and managing sales orders to coordinating therapy product preparation and delivery.

Itransition defined the target architecture for the solution and implemented a Salesforce-based platform that enables comprehensive therapy management, integrating multiple internal and external systems to support real-time alignment across clinical and operational workflows.

Platform selection

After gaining a deep understanding of the initiative's context and working in close collaboration with the customer's team, we helped structure and formalize the end-to-end therapy management process. Itransition considered 2 options for the platform foundation based on Salesforce and Microsoft and presented to the customer's our vision for the future architecture based on both solutions. The customer eventually selected Salesforce due to their existing strategic partnership with Salesforce, the platform's strong support for CRM-driven workflows, and the HIPAA-compliant Health Cloud module with healthcare-specific data models and workflows. Additionally, Salesforce provided ready-made and flexible integration capabilities via the MuleSoft solution to enable real-time data exchange with external EHR systems, which was a critical requirement for this project.

Architecture & integration

Itransition designed the solution as a real-time orchestration platform connecting all key participants - treatment facilities, order processing hubs, logistics providers, and financial systems, enabling real-time data exchange and process synchronization. To enable that, we integrated the solution with multiple platforms, including:

  • The Epic EHR system leveraged in treatment facilities, utilizing the HL7 FHIR interoperability protocol to synchronize patient data and clinical statuses
  • Internal order processing system of the customer's manufacturing units responsible for treatment material preparation
  • Solutions of the logistics providers the customer worked with, responsible for therapy product delivery to treatment facilities
  • The customer's accounting system for invoicing and payments

Salesforce Health Cloud served as the primary component for patient data collection and management, while Salesforce Service Cloud supported key entities management (patients, treatment centers, order processing hubs, etc.) and ordering and orchestration processes (treatment date selection, availability slots management, order status tracking, etc.).

Functionality overview

The delivered Salesforce-based HIPAA-compliant solution orchestrated the full lifecycle of therapy delivery, including purchase and sales order management, patient data aggregation from EHRs, coordination of treatment material preparation and delivery, as well as triggering accounting processes.

The solution key features include:

  • Real-time data synchronization between EHR, order processing, logistics, and accounting systems
  • Dynamic treatment material preparation scheduling aligned with patient treatment readiness
  • End-to-end order visibility through real-time status updates, notifications, and customizable dashboards that allow stakeholders to swiftly assess order status and identify orders that require attention
  • Validation controls to prevent duplicate patient accounts, treatment data changes, or unsafe treatment scenarios

Order initiation & patient account creation

  • Receiving the purchase order request from the treatment center's EHR with patient, clinical, and order information
  • Automatically creating a patient account, linking it to the treatment facility, and populating the patient health record using EHR data (e.g., demographics, diagnosis, treatment history, upcoming treatment information)
  • Validating patient records to prevent duplicates and ensure the patient has not previously received the therapy

Sales order creation & order processing hub assignment

  • Generating a sales order from the purchase order and linking it to the related treatment facility
  • Tracking and validating the requested treatment date with the treatment facility to ensure no changes have occurred and notifying the relevant stakeholder of any changes
  • Automatically selecting and assigning a corresponding order processing hub for treatment material manufacturing based on predefined rules (contracts with a specific treatment facility, geographic proximity, availability, capacity). In case the selected order processing hub rejects the order, the system reassigns it
  • Continuously syncing patient status from the EHR to keep track of any changes and updates that might affect the material preparation timelines

Order processing & coordination

  • Moving the sales order to the order processing stage, starting a 24-hour countdown within which the treatment material must be prepared and delivered to the treatment center
  • Exchanging real-time status updates and notifications on treatment material preparation with the treatment facility's EHR system, ensuring alignment between clinical teams and manufacturing hubs

Order delivery & invoicing

  • Initiating delivery upon treatment material completion
  • Tracking and sharing delivery status with the treatment center's EHR in real time
  • Completing the order upon confirmation of successful therapy material delivery to the treatment center
  • Triggering invoice creation in the customer's accounting system
