We developed an all-in-one solution for the customer and their end-clients to address care workflow fragmentation, streamline visit, health care, worker, and payroll management, and support their future growth.
Challenge
Our customer is a UK-based company specializing in care management services with extensive experience in care delivery. For their business operations, the customer had used multiple specialized tools, each tailored to specific workflows such as patient management, scheduling, or compliance but none offering a comprehensive enterprise solution. While trying to combine different tools to achieve operational efficiency, they faced the following issues:
- Unstable legacy systems which could negatively impact business operations and disrupt them for days
- A strong need for a complex, flexible, and efficient scheduling system driven by the combination of dynamic caregiver availability, different skill requirements, patient-specific needs and preferences, and geographic complexity
After realizing that no existing care management platform could fully meet their needs and confirming through industry networking that this fragmentation was a common challenge for other care providers, the client decided to create an all-in-one solution to consolidate the most effective features of existing tools while overcoming their limitations. The envisioned solution would address the operational challenges of care workers within their own organization and potentially benefit other companies in the sector. At the same time, the customer wanted to ensure the project’s cost-effectiveness and get a solution that would provide long-term value. However, lacking the necessary technical expertise, they needed a trusted software engineering partner to bring their idea to life.
Recognizing Itransition’s extensive experience in healthcare software development and our proven track record of delivering powerful yet cost-efficient solutions, the customer decided to entrust the project to us. Another key factor behind the customer's decision was the availability of Itransition’s proprietary telehealth and care management platform. Recognizing the growing healthcare organizations’ demand for faster project delivery driven by rising patient volumes, regulatory pressures, and the shift to value-based care, we developed a healthcare automation platform that significantly accelerates project launches. By automating repetitive tasks and standardizing initial configurations, the platform enables us to deliver projects more efficiently, uphold the highest standards of quality and security, and reduce our clients’ development costs.
Solution
Leveraging our expertise, particularly within our Healthcare Center of Excellence, Itransition helped the customer design and develop a unified care management platform. The resulting solution was designed as a B2B platform, enabling care providers to manage patients, staff, schedules, and visits through a centralized portal.
Built using .NET and React and hosted in Microsoft Azure, the platform was designed with scalability in mind to support the adopters’ future growth and seamless integration with other care providers. The platform features a scalable, multi-tenant architecture that ensures data segregation by assigning a dedicated database to each care provider. It also includes automated onboarding workflows, enabling care providers to get started quickly, and allows adopters to customize a wide range of platform settings to suit their specific needs.

The solution’s core functionality was built using Itransition’s own telehealth and care management platform, which offers comprehensive care management functionality alongside advanced security features.

The healthcare automation platform we used had a pre-built user interface, which eliminated the need to develop the portal from scratch and covered several essential functionalities. While some UI customization was still necessary to meet specific customer requirements, the process was significantly simplified.Â

Mobile app for caregivers
To support caregivers in delivering efficient, high-quality care, we developed a user-friendly mobile application. Built as a cross-platform app with support for both iOS and Android, it empowers caregivers to manage their daily responsibilities seamlessly while on the move.
The mobile app enables caregivers to perform essential tasks anytime, anywhere, including:
- Viewing schedules and visit details, along with associated task lists
- GPS-enabled check-in and check-out for accurate caregiver location tracking and visit time verification
- Task completion tracking in real time
- Incident reporting directly from the patient’s home
The platform’s regulatory compliance
Given the critical importance of data protection and system security in the healthcare sector, we needed to ensure that the platform complied with the requirements of the Cyber Essentials standard, a UK government-backed framework designed to help organizations protect against common cyber threats.
To ensure the platform met the requirements of both Cyber Essentials and Cyber Essentials Plus certifications, we followed a two-stage compliance process. First, we helped the customer complete a detailed self-assessment questionnaire, verifying that key security controls were in place, such as secure system configurations, access restrictions, malware protection, and timely software updates. This allowed us to establish a strong foundation of security practices. In the second phase, the platform underwent an independent technical assessment to achieve Cyber Essentials Plus certification. This involved hands-on testing of the developed system, including vulnerability scans and real-world simulations to validate the effectiveness of our protection measures. Special attention was given to securing remote access and safeguarding sensitive healthcare data. Successfully completing both levels of certification reinforced the platform’s credibility and ensured it met the cybersecurity expectations of care providers and regulators alike.
