Itransition developed an AI-powered add-on for Microsoft Cloud for Healthcare to streamline and accelerate encounter notation during doctor visits based on our experience with clients like Terumo, Norstella, Nuvara, MAP Medical, and Maxmed Healthcare.
Context
Itransition is a software development company delivering IT services that help our customers reach their business goals. In the healthcare domain, we have worked on dozens of award-winning projects curated by our Healthcare Center of Excellence that accumulates industry knowledge and allows us to deliver result-oriented solutions for healthcare management, ERP, CRM, telehealth, m-health, and IoT. A member of the Microsoft Partner Network since 2008, we used the experience gained through this partnership and while working with healthcare companies to develop many solutions including a telehealth and care management solution that automates a wide range of patient care management functions using Microsoft technologies.
Through our work, we noticed healthcare providers struggling with excessive administrative tasks, which reduced valuable patient interaction time. Manual documentation of encounter notes was a major pain point, taking a lot of time and impacting workers’ efficiency. To address this, we created an AI-powered add-on for Microsoft Cloud for Healthcare to streamline and accelerate encounter notation during doctor visits.
Solution
Itransition developed a voice-enabled AI tool using Microsoft Teams and Azure OpenAI and made it easy to integrate with Microsoft Cloud for Healthcare. The AI clinical note automation solution developed by Itransition achieved approval on AppSource, Microsoft's marketplace for certified business solutions, that validated the app’s compliance with Microsoft’s quality, technical, security, and performance standards. AppSource’s approval guarantees seamless integration with Microsoft platforms such as Office 365, Dynamics 365, and Azure, reducing procurement risks and ensuring smooth user experience. Furthermore, the add-on is compatible with healthcare solutions within the Microsoft ecosystem, ensuring visibility, interoperability, and growth potential.
Itransition’s team used our expertise in AI technology, healthcare workflows, and regulatory requirements to create the solution. As Microsoft partners, we have in-depth knowledge of their technological stack, which also made the implementation process easier.
We conducted a comparative analysis of various AI implementation strategies, assessing different factors such as data security, compliance, model accuracy, and interoperability with EHR systems. After research and testing, we selected an efficient and scalable approach— ensuring our AI-powered solution enhances clinical decision-making, operational efficiency, and patient outcomes while seamlessly integrating into existing healthcare ecosystems.
The app helps healthcare providers with a high administrative workload automate precise patient encounter documentation. The tool captures teleconference transcript data and uses it to create patient encounters that are automatically converted into SOAP (Subjective, Objective, Assessment and Plan) notes.
Processing with the help of AI involves:
- Recording the transcript of a doctor-patient interaction during a Microsoft Teams call
- Processing the recorded transcript
- Extracting key conversation details
- Organizing data into the SOAP format
- Summarizing the encounter and documenting the results of the doctor’s visit in a report

Because data is automatically added to predefined fields in the medical record, doctors only have to review it, make corrections, and accept the note. Once finalized, the encounter is closed, and the completed note is saved in the patient’s medical records.
