The idea of the project was to create a remote-area medical assistance system by establishing health telematics communication with the possible use of satellite telecommunication between the Gomel (city in Belarus) Specialized Dispensary and a relevant World Health Organization (WHO) Collaborating Centre, such as the WHO Collaborating Centre for Research on Thyroid Diseases in Nagasaki University, School of Medicine. The system should be implemented in a form of web application, available 24x7x365 from any point of the world. This project was the first remote medical assistance system in Belarus and Europe.

Our main objectives in the context of this project were:

  • the follow-up system establishing for patients with thyroid cancer;
  • the clinical database development;
  • the information center establishment for experience exchange and promotion of training and education of medical support of consulting services, provided by recognized specialists in this area from all over the world. 

Telemedicine project has a classic n-tier component-based architecture, which integrates both our own services and external products and services. Three layers of the system are business logic, the presentation layer and the persistence (database) layer. Such division provided simultaneous work of the design group and programmers in our project team. Cutting-edge J2EE technology enabled iTransition to develop quick and stable solution in a short time period.

The system uses sophisticated access control services, which allow the administrator to define privileges and control access to every particular service for different users/user groups/user roles.  The system is capable of performing an audit and tracing user actions and storing the information in special logs.

The functionality includes medical data collection and processing, teleconsultation process, medical data analysis and reporting.  We implemented the set of forms which contain both patients’ general information (their addresses, phones etc) and a wide spectrum of medical data and diagnoses. In addition to that we developed the unique hierarchy of documents and a convenient navigation mechanism. The universal mechanism of accumulation and storage of medical observation was developed.   

The system features as follows:

  • Dynamic medical documents form construction;
  • Windows Application Interface Emulation within the limits of Internet Application;
  • Data transfer process optimization through the network;
  • Mechanism of background processing resource-intensive tasks;
  • Medical images processing;
  • Data export/import;
  • Data confidence and integrity mechanism;
  • Multilangual support.

Technologies used: J2EE platform, JSP. JMS, EJB, Reflection API, JNDI, Java Security API, JavaScript, HTML, Java, Oracle 8i, Weblogic 5.1, RUP, MS Project, Rational ClearQuest.

Efforts: 8 man-months.
